Dear Ms Elizabeth,
Re: Ms Mandy Chang
DOB: 25/09/1962
I am writing to refer Ms chang who is suspected of having a myocardial infarction. She requires further management and enforces the importance of exercise through the health education.
Ms. chang presented to our Emergency Department on 23 September 2020 with
complaints of chest pain, sweating along with pain in the right hand. On Examination, her Spo2 was 90% on room air with unremarkable other vital parameters. A Series of laboratory investigations and radiology results have revealed a block in her right coronary artery; therefore, two stents were inserted.
Postoperatively, her condition progressed well. Even though her blood results were increased significantly which was managed with medications.
Mrs Mandy is a widow who lives with her niece. She has Type 2 Diabetes since 2018, which is well managed with Metformin. She is allergic to penicillin. She does not perform any physical activity.
It would be appreciated if you could encourage Mrs Mandy to perform regular physical activities and educate the importance of the same. Kindly provide rehabilitative care and further management. Please note, all pertinent details have been enclosed with this letter.
