Re: Mr George Gale,aged 85
Mr Gale,who was diagnosed with urinary tract infection and confusion requires ongoing care and following the discharge today.
On 10may 2021, Mr Gale brought by ambulance with the complainants of confusion and disorientation following a fall and being there for five hours.On examination,blood pressure of 155/80 mm of hg and temperature 38.1*c noticed while other parameters were unremarkable. Urine analysis revealed the above diagnosis for which he was commenced with intravenous antibiotic therapy (Amoxicillin 750mg Thrice daily)
Mr Gale has osteoarthritis and hypertension for which he is on treatment with Tab. Paracetamol and Tab.Felodipine 5mg( daily) respectively. In 2019,eventhough he diagnosed with specific colitis no treatment were prescribed other than monitoring. He lives alone in a flat as he is a widower and no family around him to look after.
Based on aforementioned circumstances monitoring of his health condition and indipendancy is profoundly beneficial. Encouraging physical activity for 30mts per day is highly beneficial for his future health.
