Dear Dietician,

Ms Patricia Cooper, who had an episode of choking, requires your urgent assessment and further management to improve his swallowing function.

On 21 March 2019, she had an episode of choking during consuming breakfast. Consequently, she was distressed, coughed and the obstructed piece was removed. Hospitalization was not required as she regained to normal. In addition, she has been taking Keflex 500mg due to chest infection.

Ms Cooper has had diabetes mellitus for years and non-adherent with diabetic diet. On observation, she gained weight about 8kgs in the last 5 months along with increased appetite. It is noted that, occasional throat-clearing following consumption of food item with milk content. Additionally, she is wearing lower and upper dentures.

Regarding medical history, conservatively managing for ischemic heart disease, osteoarthritis, severe dementia, and stroke.

Based on the aforementioned circumstances, nutritional status needs to be monitored, and performing swallowing reflexes would be imperative from preventing high risk of aspiration.
