Re: Mrs Rachel Andrewson, aged 60
I am writing to refer Mrs Andrewson, who is diagonises with renal failure and no hemodialysis since 5 years requires advice regarding on renal and healthy diet.
Mrs Andrewson admitted to the hospital on 03 September 2010 with the complaints of difficulty in breathing along with fatigue and giddiness. Mental status was not stable. On assessment noted edema all over the body and vital signs were within the normal whilst in our care taken to hemodialysis. Additionally, she is allergic to penicillin, seafood and nuts.
During the hospitalization she has been advise for fluid one litre per day apart from their encouraged sodium restricted diet. Due to unhealthy dietary pattern decreased her weight. His food preferences are fluid items along with sausage and eggs.
Socially Mrs Andrewson lives in Mart Royal Nursing home since 2002. She has had hypertension, diabetes for 12 and 2 years respectively in which she is on regular medications.
In view of the above encourage Mrs Andrewson to take high protein diet and advise to continue fluid restrictions. Kindly provide her simple recipes of protein diet.
