Dear Sir/Madam,


Re: Mr Phillip Satchell, aged 73


I am writing to transfer Phillip Satchell inti your care for the improved access of multiple sclerosis treatment. He requires further management and care at your facility.


Mr Satchell’s multiple sclerosis condition currently stable but had frequent relapses therefore was being constantly monitored for relapses. He also has Type-2 diabetes with chronic left and right leg ulcer. Filled with pus++ though has poor complaince with oral diabetic agents and diabetic diet despite being provided with education. Diabetes has been managed with metformin three times daily while leg ulcers treated with intravenous antibiotics as well as dressings twice daily.


Mr Satchell lives alone since the death of his wife in 2014 and reported to be lonely and isolated but had no mental illness.


He reported to have good relations with his two sons though they rarely meet.


May you kindly switch intravenous, antibiotics to iral and continue same diabetic medications and dressings. Please refer him

To Prince Wales Diabetic Clinic for medication review and more education. May you facilitate the referral to neurologist for Multiple sclerosis followup via you doctors and link him with community service for improvement in coping and social network.
