Re: Mr John Alen Frederick, DOB: 18 June 1987
I am writing on behalf of Ms Norah for her husband Mr Frederick. She requires a home visit to improve her husbands mobility, as he is recovering from a left knee injury.
On 12 July, Mr Frederick had a fall while climbing the stairs due to severe headache and sustained injury to the left knee. He was examined by his physician and was referred to Raymound Health Care Agency for daily dressing for the ledt knee and assissrance to meer activities of daily living.
While in our care, Mr Frederick was able to walk short distance with the aid of walking stick or assisted by someone but complains of mild pain while walking.
Socially, Mr Frederick is an auditor in a export company, married and lives with wife. He survived stroke in 2016. Since then he has slurred speech and severe weakness but walk with limp.
It would be appreciated, if you could visit Mr Frederick at their home to provide assistance and guidance to improve mobilization.
