Dear Mrs Margaret,
Re: Mrs Natasha Dobby, aged 62
I am writing on behalf of Mrs Natasha Dobby's husband's, requesting a physiotherapy services at home to support her in attending greater mobility following a fracture.
Mrs Dobby was admitted to hospital on 16 July 2012 with the complaints of injured hip following a fall. Subsequently, shifted to specialized geriatric orthopaedic rehabilitation ward. Exercises which promote his strength and speedy recovery was encouraged and rehabilitation treatment has been initiated. On 23rd July, Mrs Dobby was ambulated is possible with a support of stick for short distance.
Mrs Dobby lives with her husband in their own house. She has a history of osteoporosis. Mrs Dobby husband's is unable to walk without stick so that he having difficulty to support his wife for walking; therefore, requesting a physiotherapy service at home.
