Dear Ms Roberts

Re: Ms Jasmine Thompson, DOB: 01.07.1942

Ms Thompson, who has been recuperating from total shoulder replacement, requires appropriate care and management. She will be referred to your service today.

Upon admission, Ms Thompson has been diagnosed with right shoulder osteoarthritis and she reported chronic pain of the right shoulder. Consequently the movement and ability to carry out activities of daily living were significantly compromised. During her hospitalization, panadeine forte was prescribed and administered for pain control. She underwent total shoulder replacement on the 11 July 2017 which was successful.

Pain was successfully managed with analgesics and cold compressions. Currently, surgical site is and dry as well as drain site. In addition, the physiotherapist assisted with exercises such as neck movement ranges together with elbow and hand pendulum. Education on post surgery care will be provided upon discharge.

Based on the above, Ms Thompson needs continuous care and management after surgery. Staples will be removed 10 days after the operation in Orthopaedic Joint and Replacement Outpatient Department. Additionally, physiotherapy is recommended twice a week together with hydrotherapy once weekly. Showering assistance and prevention of deep vein thrombosis would be profoundly beneficial.

If you have any queries, please do not hesitate to contact me.

Yours sincerely

Nurse
