Re: Mr Walter Pitman, aged 69
Mr Pitman, requires a full cardiac assessment and management of hypertension which has been diagnosed since 2008. He is being referred to your service today.
Upon admission on the 01 July 2018, Mr Pitman presented to Newtown Hospital with accidental cut injury that he sustained when using the wood cutting saw. He had a deep laceration on the lower left arm which was cleaned, sutured and dressed. In addition, intravenous antibiotics were commenced and their course was completed the following day whereby oral antibiotics were started together with regular dressing.
On the same day (02.07.18), blood pressure was 140/90 mmHg and 180/90 mmHg in sitting and supine positions, respectively. He was discharged on the following day and advised to visit outpatient clinic. He presented with severe pain and mild hematoma at the clinic a day after he was discharge (04.07.18). Pain management was done as well as blood pressure was recorded and it was elevated. For this reason, appointments were done on alternative days for wound dressing and blood pressure monitoring.
Presently, on the 14 July, the wound healed well. However Mr Pitman’s blood pressure has been consistently elevated. Based on the above circumstances, cardiac assessment for hypertension would be profoundly beneficial.
