Re: Ms Jeni Fishers,aged 80
I am writing to update current health status Mrs Jeni Fishers who requires ongoing care and recovering from acute ischemic shoke.
Mrs Fishers admitted our hospital in 21 August 2021 with the complaint of syncope as well as she had the same symptom three times in last week. On assessment her vitals were normal. Hence, she had palpitation. Therefore she has performed electrocardiogram which revealed trachy cardia and brady cardia syndrome. For which pacemaker was inserted next day.
On 24 August 2021, Mrs Fisher’s MRI done which revealed the above mention condutionz. For that she was treated with thromblysis. Today, she is fit for discharge, hence she has right side weakness as well as slurred speech with constant progress.
Therefore, he has been provided physiotherapy for her weakness along with speech therapy and language therapy for slurred speech which needs to be continued.
It would he greatly appreciated if you could provide continued care and treatment for Mrs Fishers. It is important to note that she requires occupational therapy to improve her mobility and daily living activities. Please note, she has appointment of 22 January 2022, as well as she needs to be accompained.
