Re: Mr Walter Pitman, aged 69


I am writing to update you the current health status of Mr Pitman, who has been under oas care for the treatment deep laceration on the left lower arm. He requires further assessment and management of his hypertension.


On 01 July 2018, Mr Pitman was admitted to the Newtown Hospital following an recidental cut injury. Consequently he was managed with wound suturing and prophilactic antibiotics. On admission his blood pressure was 140/90mm of Hg on sitting posture and 180/90 mm of Hg on Supine position. After two days he was discharged with an achieve to attend in our clinic for following.


During his subsequent visits, even though his wound is healing bother, the blood pressure was remarkably elevated. On todays, assessment the BP is recorded with 190/84 mm of Hg and 196/86 mm of Hg in sitting and standing postition respectively.


Mr Pitaman has had hypertension since 2008. He leads a sedentary life style, also his current Body mass index under a 30kg/m cube.


Based on the aforementioned circumstances, a full cardiac assessment and appropriate management of hypertension would be profoundly beneficial for Mr Pitman.
