Re: Mr Nasser Ali, DOB: 04 February 1964


I am writing to introduce Mr Nasser Ali; a-48-year old Afganistan refugee, who was diagnosed with myocardial infraction and has undergone a coronary artery bypass graft operation. He and his family require your support to go back to their normal life.


Mr Ali and his family reached in Australia in 2010. He is married and has seven children with in the age group of 6 months to 22. They live in a rented home and do not have any family doctor. Mr Ali works as a Taxi-Driver and has only basic english knowledge.


On 9 February 2012, Mr Ali was brought to the hospital in an ambulance with the symptoms of myocardial infarction. Consequently he was managed effectively with angioplasty surgery along with medical treatments. Currently he attained a good recovery however he is depressed regarding his condition.


Mr Ali has a habit of smoking and he is overweight. He also has high blood pressure.


Based on the aforementioned circumstances your assistance for Mr Ali to cope up with his current situation would be profoundly beneficial. A follow-up appointment has been scheduled with Dr R Ling (Cardiologist) on 26 of this month. Kindly arrange a dietician to provide dietry guidance. Ofnote education regarding the importance of smoking cessation and available quit smoking programmes as recommended.
