Dear Nurse,

Re: Mr Amir Akbari, Aged 41

Mr Akbari, who was diagnosed with Gullen-Barre Syndrome, requires rehabilitation care. He is being transferred into your facility today.

During hospitalization, sudden fluctuation of worsening and recovery of the condition were noted, and his mobility compromised after an event of deterioration. Currently, Muscle tone and strength are refused and walks slowly with a frame. Also, activities of daily living is maintained. Therefore, daily physiotherapy sessions including self exercises have been commenced and plasmapheresis was performed. Depression regarding prospects was sought.

Mr Akbari has post trauma stree disorder and easily gets depressed and anxious. He lives with wife and has two children.

Regarding the aforementioned circumstances, encouraging self exercises and continuing physiotherapy are beneficial. Ensuring awarness of social worker regarding family pressure and followup are needed. A discussion about depression and psychological assessment need to be considered.
