Dear Charge Nurse,


Re: Mr Amir Akbari, age 41


I am writing to transfer Mr Akhbari who has been diagnosed with Guillian Barre Syndrome, requires regabilitation care following the discharge today.


During hospitalization Mr Akbari’s condition was deteriorated; therefore was transferred to intensive care unit and ventilated. On subsequently days his muscle tone strength was decreased and he required assistance to perform daily task, now he is able to walk with the frame. Mr Akbari has regular plasimapharesis and IV gammaglobulin for five days.


Socially, Mr Akbari is Iranian Muslim, married and lives with wife and children. He works at the service station. Couple are pursuing Doctor of Philosophy. He has had post traumatic stress disorder due to war in mid 80’s and probe to anxiety and depression. Currently, his wife is not able to cope with pressure associated with finance study and childcare. This making hum depressed and he is worried about the progress.


Upon discharge Mr Akbari needs to continue physiotherapy along with own limb exercises. His oxygenation is stable at present however requires observation for further respectively distress. He may need psychiatric assessment. So please distress with your team. In addition involve social worker assisstane to reduce wife’s stress. Finally please note that he requires halal diet.
