Dear Dr Laricrell,

Re: Mr Walter Pitman, aged 69

I am referring Mr Pitman for a full cardiac assessment and management due to hypertension. Initially he was admitted to the hospital on 01 July 2018 with the deep lacerations on lower left arm following a Law injury which was sutured, dressed and treated with antibiotics. He was discharged on 03 July 2018.

Since then Mr Pitman has been regularly visiting our clinic for this wound dressing. Mr Pitman has had hypertension since 2007. Despite of not smoking and being non alcoholic, he lives a sedentary lifestyle with no exercise.

During each visit, wound was cleaned and dressed and its healed well without infection. However, blood pressure which was taken during each visit is a cause of concern. For example, on 04 July 2018 it was 186/89mmHg sitting and 190/86mmHg when supine. On 06 July 2018, the readinfs were 190/86 mmHg sitting and 196/88 mmHg supine. On 08 July 2018, his sitting blood pressure was 182/80 mmHg and supine blood pressure is 19/8mmHg sitting and supine was 196/86 mmHg.

On the basis of Mr Pitman’s constant elevation of blood pressure it would be appreciated, if you could further assess and manage him as per your evaluation.

Please do contact ne for further queries.

Yours sincerely,

Registered Nurse
