Re: Ms Margaret Helen Martin, aged 81.
Ms Margaret Martin underwent repeated suggestive feature of coronary artery and management from your service as she is being discharged today.
Ms Martin admitted on 15 July 2017 with suggestive feature of coronary artery disease for which she underwent proedure. Post-operatively she had bruising at catheter insertion site.
During hospital, she was reviewed by dietician and follow the diet order by him. She was initiated with physiotherapy and advised to avoid too heavy lifting for 12 weeks. Currently she is stable, however she is anxious about health problems.
Socially Ms Martin Please single and has intermittent contact with family. She preferred meals and wheel services to lead standard lifestyle. In 2008 she underwent angioplasty at first time and she was commenced with regular medication for hypertension coronary heart disease hypercholesterolaemia. In additionally she wears hearing aid for hear loss.
Kindly monitor Ms Martin complaince with medication increase and dietary regime. It is necessary to monitor her wound infection and bruises. She requires regular monitoring, arrange regular family member visit for her regular progress and our social isolation.
