Dear Nurse,


Re: Mr Amir Akbari, aged 41


I am writing to refer Mr Akbari who was treated for Suillain-Baree Syndrome, now he is ready to transfer rehabilitation for ongoing care as being discharge today.


Ms Akbhari admitted to hospital on 6 March 2011. He’s condition was deteriorated and he required vestibulation therefore he kept in ICU. He was commenced with gammoglobulin 5 days. Due to hus condition his muscle tone strength decreased, however he managed with assistance for daily activity. He was provided physio for self exercises.


Socially, he lived with wife alon with his children in rented house. Presently he is depressed regarding his illness, coping with fincial issues for children study and care.


Please ensure that Mr Akhbari continues with his physiotherapy programme and encouraging him self exercises. Discuss with regarding his psychartic assessment aditionally make a appoiment with social worker for his awareness of family isses and his wife’s stress follow up.
