Dear Dr Laricell,


Mr Pitman who has hypertension is being referred back into your care and requires complete cardiac assessment and management.


On first July 2018, he was brought to the hospital with deep lacerations on lower left arm for which prophylatic antibiotics and regular wound dressings were provided. After teo days of treatment from the hospital, he was discharged and has been commenced visit to the Nurse-Led-Outpatient Clinic. During 5 visits, wound was healed without any infection. However, blood pressure has been Constantly elevated. Currently, bloodpressure is 190/84 mmHg on sitting and 196/86 mmHg while supine.


Regarding the aforementioned circumstances, a complete cardiac assessment and management would be profoundly beneficial. His medication details were forwarded shortly.
