Dear Nurse,


Re: Ms Margaret Helen Martin, aged 81.


Ms Martin who underwent treatment for corornary artery disease, requires continuing care, is being discharged today.


On 15 July 2017, Ms Martin was admitted to the hospital following an ongoing, For which an repeated angioplasty was performed which was first done in 2008.


Postoperatively recovery was unremarkable and a bruising was noted at incision site. A low-calorie high-protein, low-cholesterol and gluten-free diet were recommended after dietician review. Additionally, physiotherapy was commenced and daily light exercises as well as walking for 15 minutes were suggested. Also, heavy lifting has been advised to cease for 12 weeks.


In terms of medical history, Ms Martin has hypertension hypercholesteromia which is controlled by medication. Also, weight gain with a Body Mass Index of 29.5 and coeliac disease were noted.


Regarding the aforementioned circumstances, signs of wound infection, medication complaince, exercise and diet regime need to be monitored. Routine review by doctor, physiotherapist and dietician and family support were encouraged.
