Dear Dr Marshall,

Re: Mr Harrison Mason, DOB: 28 December 1953

I am writing to introduce Mr Mason who has been diagnosed with right side heart failure. He is referred from pulmonary condition weight reduction teaching and health diet management purpose.

Apart from Mr Mason cough started on 14 January 2021 every morning he has yellow sputum and little wheeze. However, ankle became swelling, under my examine he is overweight around 100kg. He has developed pitts oedema and his ankles; however, jugular venous presurre raised in 5cm.

He worked as a warehouse man from he was 65 Mr Mason became frustrated and inability to daily use. He lives alone.

Mr Mason needed blood gases blood tests; however he us using home oxygen therapy from long tern. Kindly provide pneumonia vaccine. Continue antibiotics as per required. Maintain healthy diet, kindly arrange for pulmonary rehabilitation programme.
