Re: Ms Margaret Helen Martins, aged 81
Dear Nurse,
I am referring Ms Martin, who is recuperating from angioplasty requires ongoing care and appropriate management from your service following the discharge after consulting cardiologistis report.
Postoperatively, the recovery process was satisfactory. However, she has a bruise while catherization, observation for any signs of infection or bleeding are suggested. Hypertension has been managed with metropolol and Ramipril, atorvastin has been administered due to hypercholesterolemia (8.3) along with aspirin and chopidogrel needs to be continued.
She lives independently with own finicial support. Considering BMI which is 29.5 a dietician and a physiotherapist have been initiated to superwise and monitor for maintaining for her. In addition, ensure to avoid heavy lifting for twelve weeks is advised. Additionally, low-calorie, high protein, low cholestrol, gluten-free diet are recommended.
It is reported, she is concerned about returning to the home and anxious regarding health. Condition, it is noted that contact with family has been intermittent, therefore, rest of social isolation is possible, kindly arrange frequent family visits to enhance her emotional support.
Based on the aforementioned, complaince with diet regime post-operative medications, exercises are necessary to improve health condition. Regular monitoring by a doctor, a dietician and a physiotherapist are appreciated.
