Dear Nurse,


Re: Mr Gerald Baker, aged 79,


Mr Baker, who was diagnosed with left total hip replacement, is required continued nursing care and being discharged today.


Post-operatively, Mr Baker was disoriented, therefore continue observation recommended and heamoglobin was dropped but during discharge was stable so, proper monitoring is advised. In addition to that weight-bearing with use of wheelie-walker and walking to length of ward without any difficulties.


Currently, recommended aspirin 100mg and ramipril 5mg as requires was commenced and panadeine forte 2 tablets 4times per say given. Daily monitoring of medication, observe skin integrity and continue the exercise like range of motion, stretching and strengthening was advised. Follow-up with FBE and UFC blood test and staple removed in after 2 weeks(21/9). He requires wheelie-walker, wedge pillow, and toilet raiser and social worker organised two week of hire of raiser from local medical supplier.


Based on the aforementioned circumstances, requires continue nursing care and it would be profoundly beneficial.
