Dear Nurse,


I am to introduce Alicia Nesbitt who was under our care due to low birth weight and born being opioid dependetn. She is being discharged today.


During hospitalization Alicia’s Mother Ms Gemme Poole has poir bonding with her baby and often needs encouragement for baby care. As she lives with her mother and grandmother kindly educate them about infant care. It might also be helpful to monitor weight regularly and promote feeding as her weight is 2.3kg.


Ms Poole is heroin-dependent and works as a sex worker. It is noted that the mother is not in contact with Alicia’s father due to donestic violence. Therefore, social servuces approved living with grandmother’s home following her discharge. Please ensure environmental safety for the child and reduce risk by updatung the social worker. It is important to observe the mother’s psychological state coping mechanisms.


It would be greatly appreciated if you could cooperate with the drug and alcohol team to provide integrated support for the mother to prevent risks of heroine use.
