Dear Mrs Penny,
I am writing to refer Mrs Sheila Cartwright to monitor her blood glucose level and medications adherence following her discharge today.
On 14 April 2017, she was admitted to the hospital due to unstable diabetes mellitus and with a small foot ulcer, for which she has been commenced in glibenclamide and Augmentin Duo forte respectively. She also initiated daily wound dressings. Kindly observe for wiund margins, any signs of complications and review her wound healing progress.
Ms Cartwright lives alone. She reported, increased consumption if alcohol and has decreased quality of dietary patterns, Since her retirement. In addition, she needs to establish her income producing activity as her pension is low. It is important to involve her in community social service work and activities.
It would be greatly appreciated if you could educate her about overall lifestyle modification and complying with a diabetic diet. It would be beneficial if you could encourage her to perform moderate exercises to promite his well being.
