Dear Dr Dane,


Re: Mr Timothy Patterson, aged 69


I am writing to update the health status of Mr Patterson, who is recuperating from lacerations, requires complete cardiac assessment and management fro his hypertension.


Upon admission, Mr Patterson had deep lacerations on lower left arm. He was administered intravenous antibiotics and the wound was dressed and sutured well. However, on 4 July 2019, Mr Pattersin visited to the out patient clinic with complainrs of severe pain and mild hematoma. His blood pressure was elevated in both sitting and supine position at 162/80mm Hg and 194/83mm Hg respectively.


Although Mr Patterson has improved in his condition, blood pressure remained elevated. Today, Mr Patterson blood pressure is elevated on both supine and sitting position.


Regarding the aforemetioned circumstances, he was suspected to have arthostatic hypertension and a fall cardiac assessment and management would be necessary. If you have further queries please do not hesitate to contact me.
