Dear Ms Penny,
Re: Ms Sheila Cartwright, aged 69
Ms Sheila, who was diagnosed with unstable diabetes mellitus and small infected (left) foot ulcer, is requires medical and social needs and he is been discharged today.
In 14 April 2017, Ms Sheila get admitted to our hospital. During hospitalization on daily saline dressing for foot ulcer, signs of complications and would margins was observed. Therefore healing was progressed, hypoglycaemic agent (glibenclamide) and antibiotic therapy was commenced. Currently she requires daily monitor medication complaince, blood sugar levels alcohol intake, diet and exercise programme, so he need medical and social activities.
Ms Sheila was retired from administrative postition and lives alone. Therefore alcohol intake was increased and dietary quality was decreased so, periodic problems with self-administration of hypoglycaemic and suggesting involvement of community social service worker is require.
Based on the aforementioned circumstances, regular monitoring of medication and social needs of Ms. Sheila would be profoundly beneficial.
