Dear Mr Penny,
Re: Ms Shella Cartwright, aged 69
Ms Cartwright who requires continuous care for unstable diabetes mellitus and left infected foot ulcers. She is being transfered to Freemont Community Health Centre today.
On 14 April 2017, Ms Cartwright initially admitted to the hospital with a small infected foot ulcer associated with unstable diabetes mellitus, which she has had 9 years.
The Foot ulcer was cleaned with saline and dressed also it was monitored for signs of complincations as well as healing process. Her current medications are Glibenclamide, metformin and aigmentin duo forte.
Ms Carrwright lives alone in a 3 bedroom house. Her dietary intake and self-administration of hypoglycemic medication were alrered since retirement from an administrative position.
Regardinf the aforementioned circumstances monitoring of medication complaice, blood sugar levels and alcohol intake would be appreciated. Moderate exercise programme and establishment of social activities should be encouraged. It would he beneficial if you could involve a social worker to modify Ms Cartwright’s life style. Hypoglycemic agent need to be continued and make sure that augmentin should be reviewed after full course.
