Dear Ms Ortner,
Re: Me Nick Daniel, aged 68
Mr Daniel is being referred to you who has been recovering from a hip replacement and requires your help to improve his mobility.
On 13 May 2019, Mr Daniel was admitted to the hospital due to an exacerbation of osteoarthritis on right hip. At that time he had a limited movement in right hip and muscular mass 45.5 reduced. For which he underwent a hip replacement.
Post-operativeky, uneasiness in hip was found and a physiotherapy sessions were commenced by hospital physiotherapist. Now, Mr Daniel is ready for doscharge. Risk for dislocation was also noted.
Mr Daniel lives with his wife and son in a single story house.
It would he appreciated if you could educate him regarding the use of walker and crutches to improve his mobility. Weight bearing exercises need to be done and advise him to use stairs after full recovery.
