Re: Mr Peter Hendrick, DOB: 21 May 1952


Mr Hendrick, who is recuperating from heart attack, requires home visit for advice in returning to work and his normal daily activities. He is due for discharge today.


Upon admission on the 02 July 2019. Mr Hendrick presented symptoms such as constant chest pain and heaviness in both arms. He has been diagnosed with myocardial infarction for which he was treated with emergency angioplasty. Following treatment, no further chest pain and the heart status is stable. In addition, physiotherapist reviewed him and commenced Heart exercise programme. Dietician also adviced on diet for weight loss cholestrol level reduction.


Mr Hendrick, lives with hus wife and children. He has a family history of heart problems. He works as a truck driver and he was advised not to drive for six weeks after his discharge from

The hospital. Additionally, Education has been provided on the reason for heart attack and need for kifestyle changes to ensure complete recovery.


Based on the above, a home visit for Mr Hendrick, he advice in returning to work as a driver and normal daily activities would be profoundly beneficial.
