Re: Mr Craig Hemsworth, aged 52
I am writing to update you the current health status of Mr Hemsworth who is progressing from tonic chronic seizure. He requires further care and monitoring following his discharge today.
Mr Hemsworth was admitted to the hospital on 19 August 2019, following an episode of seizure, which resulted in loss of consiousness and apnea as witnessed by his wife. On examination his viral parameter were normal. He was consious but was drowsy and disoriented. In addition, bleeding from the tongue and bruises were present in his ledt arm and knees. A series of investigations were performed. His Ct scan revealed subdural hematoma on right occipital region and focal epileptic variants were present in ECG.
Therefore, he was transferred to ICU under the care of neuroconsultation and was commenced on anticonvulsdant medication phenytoin which needs to be continued along with his regular antihypertensive medication. Additionally, he was examined by opthalmologist and advised to rest the eye.
Currently, Mr Hemsworth us stable. However, he is anxious and watned about the future relapse of seizure; therefore education was provided in the couples along with importance of wearing safety alert band and to avoid driving and strenous activities. He requires regular stay and healthy diet.
Pertaining to the above you are requested to monitor his complaince to medications. Kindly note that he is due for a follow-up at the hospital in two weeks to review his anticonvulsant therapy. Please continue his care and treatment.
