Dear Sir/Madam,


Re: Mr Philip Satchell, aged 73


I am writing to refer Mr Philip Satchel who was diagnosis with multiple sclerosis, type-2-diabetes, chronic left and right leg ulcers. He requires further ongoing management from your service. He is being ready to transfer your facility.


Mr Satchell presented to community health centre in February with chronic leg ulcers. His elceterus revealed and shown the presence of staphylococcus aureus. Therefore he commencrd with IV antibiotics and met Foreman. Performed dressing for left and right leg. Monitored her blood sugar,additinally provded education regarding his poor oral diabetic medication and diet.


Mr Satchell is a widower who lives in public housing. He receives aged pension. Even though he is isolated, he is mentaly stable.


During the transfer Mr satchell switch his oral antibiotics and continue diabetic treatment. Perform dressings. Kindly refer him to Prince of Wales Diabetic Clinic for medication review and education. Please arrange a appointment with neurologist for following multiple Sclerosis.
