Re: Mr Craig Hemsworth, aged 52
I am writing to update the status of Mr Hemsworth was diagnosed with tonic clonic seizure. He requires a follow-up care from your health care facility.
Mr Hemsworth was admitted due to a sudden loss of consciousness. As he was diagnosed with tonic clonic seizure, a computer tomography was performed and which revealed subdural hematoma at occpital region.
During hospital stay, he was treated at the intensive care unit. He was reviewed by a neuro consultant and prescribed medications along with amlodepine which he takes for hypertension. After 3 days treatment at a hospital Mr Hemsworth has progressed in his condition, however, he sought anxious about further events at home. His family has been educated about discharge care.
It would be appreciated if you could take care for Mr Hemsworth until his recovery, and his follow-up has been scheduled after 6 weeks. Please review his anticonvulsant therapy. Phynytoin should be continued until follow-up. Please make sure that he is wearing an alert band and please advice to cease driving and heavy activities. Kindly monitor that he is following enough sleep and regular diet.
