Re: Ms Sheila Cartwright, age 69


I am writing to update you regarding Ms Sheila Cartwright, who was diagnosed with unstable diabetes mellitus due to small infected foot ulcer requires continuing care and assessment. She us set to be discharged today.


On 14 April 2017, Ms Cartwright was admitted with complaint of non-insulin-dependent diabetes mellitus for past 9 years for whixb treated with glibenclamide 5mg daily metformin 800mg three times daily and Amoxyllin Augementin duo forte 125mg orally two times. Additionally Daily Saline dressing the wound, monitor signs of complication reduced dietary quality on observation. She had periodic problems with self administrationnof hypoglycemic medication.


Kindly monitor blood sugar levels, encourage moderate exercises programme, social activities.
