Re: Mr Stephen Kerry,

DOB : 16 June 1963


I am writing on behalf of Jennifer wife of Mr Kerry who requires assessment and physiotherapy to improve his mobility following his recent knee injury.


Mr Kerry lives with his wife. He had cerebrovascular accident two years ago, however he is alert, mentally active, slightly slurred speech and impaired balance. On 12 July 2011, he had right knee injury following a fall from stairs. Therefore, he was assisted in Shower and dressing as per general practitioner advised.


During home visit , Mr Kerry is beginning to walk using walking sticks for support but complained of usual pain while walking and managed with analgesics. His knee injury healing well with daily dressing. So, Mrs Jennifer requested for more home visits in order to accelerate progress in his mobility and discussed with her regarding physiotherapy services.


It would be greatly appreciated if you could provide assessment and evaluation about his mobility. Kindly provide home visits according to his needs and exercise programme.
