Re: Mr Stephen Kerry, DOB: 16 June 1963
I an writing this letter on behalf of Mr Jennifer wife of Mr Kerry who is requesting a home visit far her husband in regaining mobility following his recent knee injury.
Mr Kerry referred a cerebrovascular accident two years ago. Since then he has been having impaired balance and walks with limp.
On 12 July, 2011 Mr Kerry had severe headache and then had a fall while climbing the stairs which resulted in injury to his right knee. He was examined by his physician and was commenced on painkillers and was referred to we care Nursing Agency for support and assitance with dressing and other daily activities.
Since then i have been attending Mr Kerry. His knee is healing well and no signs of infection noted. His pain is managed with paracetamol 400mg. Eventhough he has mild pain; however he us able to mobilize with the help of walking aid.
Mr Kerry is active and alert,who works as an accountant. Being house-bound could negatively affect his mental ability. Therefore, it is important for Mr Kerry to regain his mobility and move around as much as possible.
Pertaining to the above you are requested to assess Mr Kerry and provide advice regime to strengthen his muscles as per your evaluation. It also would be beneficial if you could visit him daily to monitor his complaince.
