Re: Ms Sheila Cartwright, aged 69
I am writing to refer Ms Cartwright who has been diagnosed with unstable diabetes mellitus. She requires further support and monitoring from your facility following her discharge today.
Ms Cartwright was presented to the hispital on 14 April 2017, with a small infected ulcer on her left foot; therefore saline dressing was performed daily and was commenced on Augmentin Duo Forte 125mg orally twice daily which needs to be received after completion of current course.
Socially, she is single, retired who lives alone in a three-bedroom house. She has had non-insulin dependent diabetes mellitus since 9 years which has been managed with glibenclamide 5mg; metformin 800mg three times daily. Apart from that she has increased her alcohol consumption and leading sedentary lifestyle. Sometimes she also has poor complaince with her diabetic medication.
In view of the above you are requested to monitor her complaince to medication blood sugar level, diet and alcohol intake. It would be appreciated if you could guide her to some ways of expanding her social activities and encouraging in daily moderate exercise. As a income has decreased since retirement,please refer her to social worker to undertake some income producing acitivity.
