Re: Mr Peter Dunbar, DOB: 18 March 1932
I am writing to refer Mr Dunbar who is a case of type 2 diabetes. He is now moving to your locality to live with his daughter. He requires ongoing care and monitoring for his medication complaince.
Mr Dunbar has been visiting our health centre since,September 2017 with high blood sugar for which he was advised to follow a diabetes diet and exercise regime. Apart from that he was started on metformin 500mg three times daily, but he follows a sedentary lifestyle and takes medication irregularly. Therefore, education was provided regarding the importance of regular medication.
Mr Dunbar was depressed following his wife’s demise. In June 2018 he was hospitalized due to myocardial infraction and was commenced on soralol and warfarin along with ramipril for hypertension.
Currently, he us showing signs of diabetes neuropathy. As a result he has limited mobility and is mobilizing with the help of walking stick.
In view of the above information it would be appreciated if you could monitor Mr Dunbar’s medication complaince. Kindly encourage him to reduce salt, sugar, alcohol and fatty foods. In addition please educated his daughter regarding the management of diabetic care.
