Dear Sir or Mam,
Re: Mr Alex Maydew, aged 23
Thankyou for accepting Mr Alex Maydew who requires ongoing speech therapy and physiotherapy at your facility after his transfer today. He has been recuperating from neurological injuries.
Mr Maydew was admitted to hospital following a car accident. After the prriod of eight months, recovery from coma was noticed; however, impairment of speech and mobility have significantly compromised. Later, speech therapy and physiotherapy have been commenced.
During hospitalisation, depression and slow recovery were observed due to aged care setting, as her mother believes.
Mr Maydew was a third year physical education student at the University of South who came to Australia before the accident. His mother and sister are very supporting and caring. He can continue part time online study and is able to run a computer.
Based on above mentioned circumstances, your expert appropriate carr of Mr Maydew would be profoundly beneficial.
If you have any queries please do not hesitate to contact me.
Yours faithfully,
Registered Nurse.
