Re: Mrs Jasmini Thompson, aged 75
I am writing to introduce Mrs Jasmini Thompson, who underwent right shoulder replacement requires horpery care and support from you as she is being discharged today.
Mrs Thompson was admitted with complaints of chronic right shoulder pain due to right shoulder osteoarthritis she has decreased movement and was unable to easily out daily arthritis. Consequently she underwent surgery on 11/07/2017 and her condition was stablized post operatively. Besides she was able administered medication and applied cold compress for managing pain. Both wound and drain its were clean and dry.
During hospitalisation Mrs Thompson visits as stable. However she requires assistance in daily activities due to reduced mobility. Her wound was analysed daily and provided proper care along with the application of cold compress and shoulder have even 4th hourly. She had given physiotherapy as per TSR protocol.
As per the discharge plan it would be greatly appreciated if you could able to provide discharge education regarding the importance of wearing right arm sling for avoiding heavy lifting for 4 weeks. Kindly encouraged her to continue outpatient physiotherapy and community hydrotherapy as per schedule. Please ensure the need of staplesb removed after ten days and follow-up in Orthopaedic Joint Replacement Outpatient Department. It would be beneficial if you could provide assistance in showering and administer cleavane subcutaneously four days to a prophylaxis for deep vein thrombosis.
