Dear Dr Laricrell,
Re: Mr Walter Pitman, Aged: 69 years
I am writing to refer Mr Pitman, who was admitted to our clinic on 01/07/2018, following a cut injury with a hand saw. Due to making excellent progress, he is scheduled to be discharged today and now requires an urgent cardiac assessment and management of his hypertension.
During hospitalization, he presented with a deep laceration on his lower left arm. It was cleansed, sutured and dressing was applied. He was treated with antibiotic. His blood pressure was persistently high for both sitting and suprine position through out his hospital stay, however, his wound condition is better and healing. He had a severe pain with mild hematoma with a pain scale of 8/10. He lines a sedentary lifestyle without any exercises. He has a medical history of hypertension and his BMI is 30.
It would be greatly appreciated if you do an urgent cardiac assessment for his hypertension.
Should you require further information. Please do not hesitate to contact me.
Yours sincerely
Nurse.
He was advised to visit the
nurse-led outpatient clinic.
