Re: Mr Jennifer Linda Allen, DOB: 25 1935
Mr Jennifer Linda Allen, who is recuperating from angioplasty is being referred to your facility, requires home visits for continuing care and definitive management following his discharge tommorow.
On 16 March 2019, Mr Allen was admitted to the cardiac unit of Admount Public Hospital. During hospitalisation, he has been diagnosed with angina and coronary artery disease ; therefore, he underwent surgery. His post-operative recovery was excellent ; however, bruising at the catheter site was deleted. Physiotherapy has been commenced with light exercises which includes 15 minutes walk. Low calorie, high protein, low cholesterol and gluten free diet was recommended as per the dietician's advice.
Mr Alen,divorced has no children and no contact with family or society. He has had hypertension, hypercholesterolemia and coeliac diseases for which he is on aspirin, clopidagered, ramipril atorvastatin and metoprol. His body wieght is above the recommended level

It would be profoundly beneficial if you could moniter medication compliance, exercise, dietary regime, follow-up appointment to physician as well as wound site for signs of infection. Please note, refraining from heavyweight lifting is significant for a fort night. It is essential to arrange family support in order to avoid social isolation and to facilitate progress in general condition.
