Re: Ms Sally Fletcher,DOB:25 October l1993
Ms Sally Fletcher, who has been recuperating from laparoscopic surgery is being referred to your service for on going care and thorough management following her discharge.
Ms Fletcher presented with the complaint of frequent acute menstrual pain at the left lower quadrant along with
Occasional constipation which has not responded to naproxen.Trans- vaginal ultrasound revealed 6cm cyst;therefore, she was diagnosed with endometriosis. Consequently, laparoscopic surgery was performed and her post- operative recovery was uneventful.
Upon discharge, Ms Fletcher was permitted to take shower from 26 March 2018.Driving can be resumed 24- 48 hours after the completion of analgesics as well sexual activity is recommended after two weeks post- operatively. She was encouraged to take oral fluids and gradually proceed to regular diet.Mobility is encouraged as tolerated.She is ready to be discharged once she is able to perform daily living activities independently. It is profoundly beneficial to arrange someone who can drive her home on discharge.
It would be appreciated if you could take over the care of Ms Fletcher and provide definitive care and appropriate management.
