Mr Martin Williams, who attempted suicide owing to depression related to gambling addiction is being referred to your facility for detailed evaluation and expeditious management.
Ms Wilson,a retired book keeper, lives with his wife Joan,used to be a keen lawn player.He had begun gambling two years ago which lead him to debt and subsequently committed suicide with Mogodol. Currently,he is ashamed and regretful as well as showing willingness to overcome gambling addiction.
Mr Wilson has had type 2 diabetes mellitus, psoriasis and depression for which he is on Diprosone OV cream and Lovan 20mg.He smokes heavily and drinks socially. His body weight is below the recommended level (BMI: 18) whereas has poor appetite and is physically unfit.
Pertaining to the above details, it would be profoundly beneficial if you could arrange an appointment in gambling therapy for Mr Wilson. It is essential to encourage in joining Gambling Addiction Action Group,Quitline Program and social sporting activities. Please note, monthly follow-up appointment with psychologist Dr Brian Murphy at Lyall MCEven Hospital is significant.please arrange a frame and wheelchair for one week. Kindly monitor medications and maintenance of low glycaemic index diet for diabetes mellitus.
