Dear Ms.Ward,

Re:Mrs.charlotte price,aged 86

Mrs.charlotte price ,who is recovering from a concussion episode.She requires home visit for her assistance and supportive care following her discharge today.

On 02 December 2018 Mrs price was admitted with complaints of unconsciousness following a fall at home.Neurological examination and CT scan were unremarkable.However , continued neurological observation for 48 hours is required.During hospitalization infected ingrown right toe nail was noticed.Wound dressing was provided.therefore, antibiotics and analgesics was prescribed.

Mrs.Price requires home visit by a community nurse twice a week.Asssisstance is required for showering and dressing.Education is required regarding personal hygiene and diet.Dressing changes required twice a week and monitor medication compliance on Amoxycillin and ramipril.

Mrs.Price lives alone, a retired postal worker.she wants to live independently.She was using walking stick due to risk of fall.She has had hypertension past 25 years . She smokes heavily and occasional drinker ,as well as, urinary incontinence also, scoliosis.She was on medication ramipril 10mg for hypertension and brufen as required due to scoliosis.Inaddition to that Amoxycillin 500 mg three times daily for 2 weeks was prescribed for toe infection.

Based on abovementioned circumstances, regular home visits ang ongoing care for Mrs.price,it would be greatly appreciated
