Dear Ms Bmin,
RE: Mr Gerald Baker, 79 years old.
I am writing regarding Mr Baker who was admitted to our hospital on the 03/09/2010 for a left total hip replacement. Due to making excellent progress he is scheduled to be discharged today and now requires your continued care and management.
During hospitalisation, he was treated with three units of red blood cells due to dropped heamoglobin. He was then prescribed ramipril for his ongoing hypertension as well as aspirin and co-codamol for pain relief. His incision site was dressed daily, and ranges of motion, stretching and strengthening exercises were given under guidance by the occupational therapist. Please note, Mr Baker can walk the entire length of a ward without difficulties with a wheel walker which we have provided for him along with a wedge pillow.
It would be greatly appreciated if you could monitor has medications and his haemoglobin level. Also, please assist him to maintain his skin integrity and kindly encourage him to continue his exercise program. The hospitals social worker has organised a two-week hire of a toilet raiser from the medical supplies for him.
A follow-up appointment has been arranged for Mr. Baker to have his staples removed and to have full blood examination,(FBE), urea,electrolytes, and creatinine tests,(UEC) at the city hospital on the 21st September.
