Re: Ms Margaret Helen Martin, aged 81
Ms Margaret Helen Martin, who had an angioplasty today, requires further care and evaluation via home visits from your service, following the discharge today.
Ms Martin was admitted with angina; consequently, the diagnosis of coronary artery disease was ruled out. Postoperatively, or fluids and light foods were recommended. Apart from bruising, no other complications over incisional site were noted. Concerns for managing the activities of daily living and health following the discharge have been identified.
Ms Martin lives independently and her relatives visits occasionally. She prefers meals on wheels. Besides, she has been on aspirin and clopidogrel since 2008 after the first angioplasty. Also, she takes metoprolol, ramipril and atorvastatin for hypertension and hypercholesterolemia, respectively. Her current BMI is 29.5.
Based on the aforementioned circumstances, assessment for medication - compliance and wound site would be profoundly beneficial. Small and frequent diet including high - protein and low - cholesterol as well as adequate amount of fluids have been advised. Light exercises needs to be performed regularly, under the physiotherapist's supervision. Abstinence of lifting heavy objects for next 12 weeks have been suggested. Family needs to be counselled for frequent visits to reduce the complication of social isolation.
