Re: Ms Margaret Helen Martin, aged 81
I am writing to refer Ms Martin, who is having coronary artery disease and underwent recent angioplasty. She is being discharged tomorrow and requires your further care and management at home.
Post operatively Ms Martin had bruising at the site of catheter insertion. She was well tolerated to light diet and fluids. But, she is having conditions regarding self care at home.
At socially, Ms Martin lies independently. She has infrequent communication with other family members. She retries on food delivery service such as Meals on Wheels.
Medically, Ms Martin has had hypertension, cocleae disease hypercholesterolemia and on medications. She also had angioplasty in 2008. She uses hearing aids for hearing loss.
Upon discharge Ms Martin is adviced is concern low calorie low cholesterol, high proteins and gluten free diet which must be supervised by designated dietician. She is also adviced to take frequent small meal and snack along with plenty of fluids. In terms of her physical activity, she is advised to avoid heavy lifting and irritated to perform daily light exercise such walking as per plans of physiotherapist.
It would be greatly appreciated if you could arrange home with daily, for wound site observe and is follow medication, exercise and dietary regime.
