Dear Nurse-in-Charge,
Re:Ms Margaret Helen Martin,DOB:25 July 1935
Ms Margaret Helen, who underwent repeated angioplasty, requires home visits regarding further care and support following her discharge tomorrow.
Yesterday, Ms Martin was admitted to hospital with angina secondary do coronary artery disease. Consequently she has undergone the procedure.Post -procedure period was unremarkable. Despite having bruisness at catheter site, no signs of infection or bleeding were noted.she tolerates liquid and soft diet as well. Currently, she has concerns regarding return back to home .As a result home visits were recommended.
Ms Martin has the history of coronary artery disease, hypertension and hypercholesrolemia for which she takes aspirin ,clopidogrel,metoprolol, ramipril and atorvastin respectively.
Based on the aforementioned circumstances, regular assessment of the wound site for bruising and infection would be profoundly beneficial for Ms Martin. It is imperative to monitor medication adherence as well as dietary regimen. Mild exercise such as15 minute walking is recommended;however, refrained from heavy lifting for 12 weeks.Regular Supervision by the dietician and physiotherapist are recommended.
