Dear Sir/ Madam,


Re: Ms Margaret Helen Martin, aged 81


Ms Martin, who is recuperating from angioplasty, request home visit for further care and evaluation from your service, following the discharge.


On 15 July 2017, Ms Martin admitted to Greenville Public Hospital with Conplaints of angina and diagnosed with coronary artery disease. Post-operatively, able to tolerate fluids and light diet. Although, bruising at catheter insertion was present, no signs of infection or bleeding were noticed. Therefore, feels anxious to return home and finds difficult to cope with the current situation.


Ms Martin, lives in own house in Greenville. She has had coronary artery disease, hypertension and hypercholestarolemia, for which us managed by aspirin, clopidogrel ramipril and atorvastatin. In addition, overweight has observed along with family history of coronary heart disease. Hearing aid is used due to hearing loss and anxious about health. In 2008, underwent angioplasty.


Based on the above circumstances monitoring of wound site and medication complaince would be profoundly beneficial. Small frequent high-protein, low-calorie and gluten-free diet as well as plenty of fluids has been suggested. Daily light exercise needs to be performed regularly under the supervision of physiotherapist is advised. Abstinence of lifting heavy weight for 12 weeks has been suggested. Family needs to be counselled for frequent visits to reduce the complication of social isolation.
