Re: Ms Margaret Helen Martin, aged 81
I am requesting to have a home visit to Ms Margaret Helen Martin, who has been recuperating from angioplasty,requires basic care and psychological support. She is scheduled for the discharge today.
Ms Martin, has been diagnosed with coronary artery disease and angina therefore, she underwent above-mentioned surgery.
During postoperative period, she has made a significant recovery including tolerate the fluids and light diet; however, she has been noted to be anxious regarding the changes in lifestyle adaptation. Apart from this, has bruise over the catheter insertion site.
Ms Martin, had angioplasty on 2008. Additionally, she has had coronary artery disease, hypertension,hypercholesterolemia forwhich, she is on medications. Medication chart has been enclosed with this letter.Along with this, she has a family history of coronary artery disease. She is leading a sedentary lifestyle thus, (BMI 29.5 ) reveals as a overweight.
Ms Martin, is a single, who resides in her own house.She has been financially independent and has limited contact with the family. She preferred to take meals from delivery services.
It would be greatly appreciated, if you could monitor her medication compliance. Kindly arrange appointments with dietitian and physiotherapist to assist in dietary pattern and mobility. Wound care will be provided to prevent infection and further complications. Your subsequent visit of Ms Martin is essential to attain a excellent recovery.
